Southwark and Lambeth Integrated Care​



Southwark and Lambeth Integrated Care is the partnership between Southwark and Lambeth Clinical Commissioning Groups, local GPs, the three local NHS Foundation Hospital Trusts (Guy’s and St Thomas’ NHS Foundation Trust, South London & Maudsley NHS Foundation Trust and King’s College Hospital NHS Foundation Trust), Southwark and Lambeth local authorities, the Voluntary and Community Sector and people in Southwark and Lambeth, supported by Guy’s and St Thomas’ Charity.

Our vision is to increase the value of care we provide for the people of Southwark and Lambeth, so they can lead healthier and happier lives.

This means communities and health and social care professionals can be supported to work better together to provide pro-active and preventative care that gives local people control of their own health and well-being. For professionals, we need to make the right thing to do the easy thing to do.


We are keen that the citizens of Southwark and Lambeth are fully involved in the development of integrated care. You might want to tell us about your experience of services and share your ideas on how they might be improved. You can help us to engage others in the community; attend our Citizens’ Forums to influence decisions; or join one of our teams of people designing and testing new service ideas. Maybe you just want to keep up-to-date with the work taking place; or there might be other ways that you want to get involved – do tell us.
