Diabetes Book & Learn service now available across south London

A new integrated diabetes education booking service called Diabetes Book & Learn has been launched for health professionals to refer people with Type 2 diabetes from across south London.

Diabetes Book & Learn can be accessed via the website www.diabetesbooking.co.uk

For the first-time, health care professionals are able to refer people with Type 2 diabetes to Diabetes Book & Learn, where they will be offered the choice of many diabetes education courses available across south London.

This will give people a much wider range of course locations and timings, potentially significantly increasing attendance rates and helping people with Type 2 diabetes to take more control over their own health and care.

This initiative has been made possible by the joint collaboration between the Health Innovation Network and ten south London Clinical Commissioning Groups, led by NHS Southwark CCG. Additionally we expect that two more south London CCGs, will join the service in November (Bromley and Greenwich).

In mid November, the service will also open up to members of the public with Type 2 diabetes to self-refer themselves and book their course online or with the help of the call centre team rather than needing a health professional to refer them for their education. The service will also be used for health professionals to refer people with Type 1 diabetes from the beginning of November.

For health professionals

An app and self-referral option will be available shortly, but until then please continue to refer patients using this self-populating form on EMIS. The same form will be available on DXS within the next week.

You are still able to refer to DESMOND by using this referral form, but please email it to diabetes.booking@nhs.net

Alternatively, you can refer by completing the online form via www.diabetesbooking.co.uk/GPrefer

GP Federations will continue to support practices with importing.
