National Prize for our work on developing care coordination for people with multiple long term conditions
Congratulations to Southwark's Local Care Networks who received an award at the Patient Experience Network National Awards (PENNA). Rosemary Watts, Head of Membership, Engagement and Equalities at NHS Southwark Clinical Commissioning Group (CCG) and Dr Louisa Dove, Chair of the north Southwark Local Care Network, were presented with the award at the ceremony on Thursday 1 March.
They were runners-up in the Commissioning for Patient Experience award for their engagement work around developing care coordination with people living with three or more long term conditions. NHS Southwark CCG supported the engagement, working collaboratively with Healthwatch Southwark and other Local Care Network partners.

The PENNA awards ceremony was a great opportunity to showcase our work and we would like to thank everyone who supported us."
The team's award-winning entry focused on engaging people with long term conditions around the delivery of co-ordinated care, which involves identifying what is important to people and developing a care plan for individuals who require a range of services and support to manage their condition.
Engagement activities were used to understand what is important in people's lives to form the basis of the care coordination work and included:
- Patient films which were shown at a learning event for GPs, Practice Nurses and other clinicians
- Patient stories were collected
- Patient workshops – we invited people with long term conditions and their carers to several engagement workshops
- Round table discussions with patients and clinicians
- Patient feedback on example letters and care plans
Healthwatch Southwark is currently seeking patient's views and stories of care coordination as part of the evaluation of the care co-ordination pathway.
