​NHS continuing healthcare

NHS Southwark Clinical Commissioning Group (CCG) provides 'NHS continuing healthcare' for people who need support to maintain their wellbeing but do not need to be in hospital.

For example this may be provided for people with learning disabilities or people who are physically frail.

Assessments for NHS fully funded continuing healthcare in Southwark

The NHS carries out assessments to determine whether a patient is eligible for continuing healthcare. This may be NHS staff who are involved in the patients care or who may particular expertise in the patients condition. 

If you would like an NHS fully funded continuing healthcare assessment for yourself, a family member or a person you care you can speak to:

  • Your social worker (if applicable)
  • A member of your clinical team involved in your care

NHS funded nursing care in Southwark

NHS funded nursing care is a weekly payment made on behalf of the residents of care homes with nursing who have been assessed as ineligible for NHS continuing healthcare but need to be in the care home with nursing so they can access nursing any time of the day or night.
